Please note that this is a summary of changes to the Blue Cross and Blue Shield Service Benefit Plan in 2021. Previously, it was $6,500 for Self Only and $13,000 for Self + One and Self & Family. For FEP Blue Focus members, we are increasing the out-of-pocket (catastrophic) maximum to $7,500 for Self Only and to $15,000 for Self + One and Self & Family contracts.Previously, you paid 30% of our allowance. We will provide benefits for continuous home hospice care at no out-of-pocket cost to FEP Blue Focus members.Previously, we provided benefits every three years. For Standard and Basic Option members, we will provide benefits for hearing aids up to $2,500 every five years.We are increasing the Basic Option copay for emergency room care to $175.We are increasing the out-of-pocket cost for tier 4 and 5 specialty drugs for Standard and Basic Option members.We are making changes to our approved drug lists (formularies).If you are a Standard or Basic Option member, you must complete the Blue Health Assessment (BHA) to receive the free blood pressure monitor. The Hypertension Management Program will be limited to the contract holder and spouse on your contract, 18 and older.You’ll now have until Decemfor the 2021 program.
